CoopAnimArt - Five dysfunctions assessment

This assessment is provided in the framework of the CoopAnimArt project, and is inspired by Patrick Lencioni's book "The Five Dysfunctions of a Team".

The primary purpose of this assessment is to provide you with a sense of your team’s unique strengths and areas for improvement. For a more accurate and holistic analysis, we encourage your entire team to complete the Assessment. While the assessment itself provides an interesting perspective, its most important aspect is the discussion it may provoke around specific issues.

The information collected as part of this questionnaire is strictly anonymous and is recorded in a computerised file, hosted by Occitanie en scène, in France, to enable your responses to be processed and used. For technical reasons, your answers will be kept for five years. In accordance with the French Data Protection Act and the RGPD of 25 May 2018, you may exercise your right to access and rectify your personal data by contacting: infocomm@occitanie-en-scene.fr.
